Special health notes for handy reference: Head Lice
Entering 6th grade parents, please remember your child must have the Tdap vaccine before the start of school if they are 11. Written proof of immunization from your doctor is required. If your physician is out of the vaccine and this has happened in the past , a doctor's note to that effect must be submitted. If your child is 10, they must have the immunization when they turn 11. No student may attend school without the proper immunizations.

Please remember your child needs a current physical to play. If your student has not had a physical in the past year, please make sure you schedule one. If your child is in grades 2,4, or 7 and does not have a current physical on file, please make an appointment to see your doctor. Medical forms are available on the website.
Parents, please remember if your child has sustained an injury or has a medical condition that prevents them from participating in Gym class, we need a physician's note stating that. Please be advised that if they are out of gym, they can not play on a sports team, or dance until they are returned to full activity.
I must have a Doctor's note clearing them medically before they can return to full activity. There are NO exceptions to this.I must follow the physician's orders. Without a Doctor's note clearing them, their activity will continue to be restricted. This is not to be punitive, this is for the health and safety of our students. I ask your cooperation in this matter.
